General Instructions

• If you have previous X-rays or scans of the area we are to study, bring them with you to your appointment.
• With all exams, take your usual medications with the minimum necessary water, unless instructed otherwise.
• Leave jewelry and watches at home. Wear loose, comfortable clothing.
• Let our staff know if you are pregnant or breast feeding.

Mammography

• No perfume, deodorant or powder on the chest or underarm area.
• You will be asked to undress from the waist up. Please wear appropriate apparel.

Breast Biopsy or Needle Localization

• You will be contacted by one of our Technologists the day before your exam.

Breast Ultrasound

• No specific instructions. Please call our center if you have questions.

What to Bring:

1. Please bring the following with you to your appointment:
   • Current Health Insurance Card
   • Driver’s License
   • Copies of your medical records
   • Copies of or original x-rays, CAT scans, MRI scans, mammograms or ultrasounds
2. If this is your first office visit:
   • Please arrive 20 minutes before your scheduled appointment to complete registration and medical form
   • If possible, bring with you a completed “New Patient Medical History Questionnaire” (this would hyperlink to printable pdf form)
3. If you are an established patient, please arrive 10 minutes before your scheduled appointment.
